In accordance with Article V of the decision of the Minister of Foreign Affairs No. 3497.3 / 550 / ΑΣ 4000 / 30.12.2005K considered the following documents and supporting documents Actions:
Unified incomplete application form (the European Commission's decision No. 354/2002) accompanied by a recent photograph of the applicant.
A valid passport or any travel document other recognized by the state be compatible with other travel documents listed in the travel documents the table by the visa which allows foreign nationals to cross the external borders of the countries in the region in the validity period which should not duration exceeding three months (Article 13 , paragraph 2 CIS).
A copy of the criminal record of the applicant is issued by local authorities.
A medical certificate issued by a public or private accredited to confirm the patient is free of all diseases that could put public health at risk in accordance with the international situation and the World Health Organization as well as any infectious or parasitic diseases and other requests to take measures to protect public health institution.
Travel which expired apply for the duration of the visa at least to cover all the expenses that may arise during the migration process or healthy for an urgent medical examination or emergency treatment in a hospital during the stay for reasons of insurance.
Apart from the above-mentioned public documents, the applicant must deliver the supporting documents referred to in the previous decision of the Minister of Foreign Affairs, specifically in articles 6 to 36 - depending on each case - the consular and political competent authorities.
